The problem is rarely the audit — it’s what happens afterwards

Most homes audit conscientiously. The audits get done, the scores get written down, and the findings get discussed. What breaks is the distance between the finding and the fix: the action is agreed in a meeting, recorded in minutes, and then depends on somebody remembering it. Weeks later the same issue appears in the next audit, and it is genuinely unclear whether the work was done badly or never started.

The second break is evidence. It exists — training records, supervision notes, photographs of a repaired handrail, a signed policy — but it lives in different places from the audit that needed it. So when an inspection is announced, the service spends its energy assembling proof of work it already did.

Your audit templates, not somebody else’s

Build the audits your service actually runs — medication, infection control, care plan quality, falls prevention, nutrition and hydration, kitchen and environment. Questions can be pass/fail, yes/no, a number or a rating, and each section carries its own weighting so the score reflects what matters. Templates are versioned, so editing one never rewrites history on an audit that has already been completed.

Scored as it is completed

Audits are scored automatically against the threshold you set, on a laptop in the office or on a phone while walking the floor. There is no second job of adding up results, and no gap between doing the audit and knowing where the service stands.

Findings become tracked actions

Anything that fails becomes a task with an owner and a due date, attached to the audit that raised it. Overdue work is visible before it becomes a pattern, and an action is only closed when someone confirms the work was actually done — not when the meeting moves on.

Evidence attached where it belongs

Photographs, certificates and documents are uploaded straight onto the audit or the finding they prove, including from a phone camera. When evidence is asked for, it is already in place and already dated.

Rolled up into governance

Results feed governance indicators against the targets you set, alongside incidents, falls, reviews and overdue work. That gives you the trends a quality meeting needs and a full evidence pack you can export in one go.

CareCommandOS scores Safe, Effective, Caring, Responsive and Well-led from your live records, against the targets you set. Each indicator opens the records behind the figure, so a low score is a list of specific things to attend to rather than a judgement you have to interpret.

That means the honest answer to “where are we weakest?” is available on any ordinary Tuesday, and the work between now and an inspection is a short list rather than a guess.
